What you'll need before you call

Before calling your broker or AXA to report a claim, make sure you have easy access to:

• Your AXA automobile policy number.
• Detailed information on the incident, including name(s), contact information, licence plate number(s) and auto insurance information of any other persons involved, as well as the police accident report number.

In case of an accident in Quebec

If there are injuries, call 911 for immediate assistance. Report the accident to the police:

• If anyone has been injured.
• If damages to any vehicle appear to be major.
• If damages have occurred to the other property.

In the event of an accident, fill out a joint report form and include the following information:

• Name(s) and contact information of any parties or witnesses involved.
• License plate number(s) of car(s) involved.
• Insurance information of any other party involved.

Make sure you always have a joint report form in your car. You can obtain copies from your broker, an SAAQ office or police stations.

In case of an accident outside Quebec

If there are injuries or property damage, call the police by dialling 911.

In the case of any accident:

• Try to obtain the accident report number with the name of the officer or the sector in which the report was written.
• Be sure to take down contact and other information on the other persons involved in the accident (witnesses, licence plate numbers, etc.).

If your windshield or window is broken

In the event of broken glass only, dial 1 877 666-1026, 24 hours a day, seven days a week. A member of the team dedicated to this type of claim will assist you.

Before contacting us, be sure to have the following information at hand:

• Policy number
• Name
• Address
• Model of vehicle

Windshield replacement may be done at your home or at a repair centre. We'll give you the address of the centre closest to your home.
